The Agreement Request Form (ARF) report is intended for agencies to request new or modify existing T/LPA agreements with NMDOT. This report will only be available for the Active STIP.
Filters
Once the user selects the Agreement Request Form (ARF) report in the "Select Report" list, these filters will appear:
| Field | Description |
| Control No. | A picklist for selecting the related Control No. |
| Procurement Vehicle | A picklist that includes the options of "New Agreement" and "Amendment." |
| T/LPA Address | A textbox that allows a physical address to be entered (limit 100 characters). |
| Person in Responsible Charge | This section allows a responsible individual to be specified through the Name, Title, Phone (number), and Email text fields. |
| T/LPA Signature Page Titles | This section allows Titles to be entered for the Agreement Signature Page. There are three text fields in this section-- Title 1, Title 2, and Title 3 (limit 50 characters). |
| Comments | A text field for the user to enter Comments related to the Agreement Request (limit 500 characters). |
| Project Funding | This section consists of seven Yes/No picklists: Include Preliminary Engineering, Include Right-of-Way, Include Utility, Include Other, Include Construction, Include Construction Engineering, and Include Planning and Research. |
Print Form
The pop-up Project Agreement Form has a "Print" button in the top right which allows the user to print the completed form. The printed version will also include two additional sections:
